What does the Parent/Guardian see?

Once the sponsor sends a Ready Records request, the parent/guardian receives an email and a text message with a link to complete the form. Instruct them to look for an email from “[email protected]” and a text from "651-300-2911". Within those messages there will be a link for them to click. The module will walk the parent/guardian through the process step-by-step.

Screen 5: Case or Eligibility Number

On this screen, the parent or guardian has the option to provide a case number or eligibility number for SNAP, TANF or FDPIR.


  • If they do have a SNAP, TANF or FDPIR number, they should select the program for which they qualify and provide a case number. They then skip to screen 15.
  • If they do not have a SNAP, TANF or FDPIR number, they should click continue.

Screen 6: Foster Child

This screen asks if any children in the household are foster children.

  • If all children in the household are foster children, then the system will skip to screen 14.
  • If one or more of the children in the household are not foster children, then they should click continue.

Screen 7: Categorical Eligibility

This screen asks if any children in the household are categorically eligible for the highest rate of reimbursement because they are legally classified as homeless, runaway or migrant. 

  • If all children in the household are categorically eligible, then the system will skip to screen 14.
  • If one or more of the children in the household are not categorically eligible, then they should answer No and click continue.

Screen 8: Option to Decline

This screen provides the option for a parent/guardian to certify that their household income is above the threshold for reduced-price meals and decline to provide further income information.


If the household checks the box that their household income is above these levels, then the system will skip to page 14 and they will not be asked about their household size or income.


If the household would like to provide household size and income information, they should leave the box "Our Household Income is Above These Levels" unchecked and click continue.

Screen 9: Additional Children in Household

The parent/guardian is now in the part of Ready Records that collects household size and income. This screen provides the opportunity for the parent/guardian to add additional children to the household.


If there are no additional children in the household besides those enrolled in the center, they should click Continue.

Screen 10: Child Income

Child income does contribute to the household income. This screen provides the opportunity to provide child income. If there is none, they should click continue.

Screen 11: Additional Adults in Household

Household size is important in determining the income eligibility category. This screen provides the opportunity to list other adult household members to get an accurate household size.

Screen 12: Sources of Adult Income

This screen asks for the sources of income for the adult members of the household. For each source of income with a "Yes" answer, the following screens with ask about the amount and frequency of income from that category.
